For Student Participants

All student participants must have current Waiver and Commitment to Character forms on file prior to participating in any CYT Phoenix programming each season. Click the title of each document to complete the form. You must complete the fields and save the document before uploading to the submission form.
 
  1. Student Waiver: The parent or legal guardian must complete and sign the Student Participant Waiver. If a student participant is 18+ years old, the student must complete the additional 18+ section.
  2. Commitment to Character: The student and their parent or legal guardian must review, initial, and sign the Commitment to Character before submitting their seasonal documents. The Commitment to Character provides a summary of the behavior standard for all participants to provide a healthy and positive environment at CYT Phoenix.
  3. Transportation/Pick Up Form : Please submit this form or bring in on first day for who is allowed to pick up the student and if they can sign themselves out.

For Adult Participants (Parents, Volunteers, and Workers)

All adult volunteers, staff, participants, and independent contractors ("Adult Participants") must have current documents on file for the items listed below. Click the title of each document to complete the form. You must complete the fields and save the document before uploading to the submission form.
 
  1. Adult Waiver: A current waiver must be on file prior to serving at CYT programming- For Adult Waiver content, scroll to the bottom of the linked PDF.
  2. Background Check Authorization: All Adult Participants must have a clear, current background check before serving at CYT. Background checks are valid for two years. 
  3. Child Abuse Safety Training All adults working for CYT or volunteering with CYT must complete the Child Abuse Awareness training with a passing score on the end of the video assessment.  The training is 1 hour long and only needs to be completed once by each individual. This will be emailed to you from Protect My Ministry 
  4. Commitment to Character: The Commitment to Character for Staff and Volunteers was developed for the safety of CYT students, the protection of CYT staff and volunteers, and to clearly communicate the practices and behaviors that are appropriate at CYT activities.
