For Student Participants

All students must have the following forms on file each season before participating in any CYT Phoenix programming. Click the titles below to complete and submit all forms at once with the button bellow.
  1. Student Waiver/Media Release: To be completed by a parent/guardian.
  2. Commitment to Character Ages 8-18
    Commitment to Character Ages 4-7 Reviewed and signed by both the student and a parent/guardian. This outlines our expectations for a respectful and positive environment.
Forms can be filled out digitally, saved, and uploaded using the submission button below. Paper versions are NOT accepted and must be uploaded digitally.

For Adult Participants (Parents, Volunteers, and Workers)

All adult participants in MAINSTAGE must submit the following documents before serving with CYT Phoenix just once. Click each title to complete. **Note BG Checks and Saftey Training only need to be done ONCE** 
  1. Adult Waiver: Required for all adults serving with CYT.
  2. Background Check Authorization: All adult participants must pass a background check.
  3. Child Abuse Safety Training: A one-time, 30 min online training with a short assessment. A link will be emailed to you from Protect My Ministry.
  4. Commitment to Character: Outlines appropriate conduct and expectations for all CYT staff and volunteers, ensuring a safe and positive environment for students.

Additional Adult Volunteer

If you are over 18 and want to volunteer at CYT, please fill out these forms and submit below: 
  1. Waiver (Adult participant waivers)
  2. Adult Commitment to Character
  3. Adult Certificate for Child Safety completion - Once you fill out your background check we will email a child safety training to you from Protect My Ministry.
